Practice Manager

Taylor Lounder

Taylor brings a wealth of experience to her role as Practice Manager, supported by a degree in Health Science and a career spanning multiple facets of healthcare. Her background includes work in long-term care, cardiology, orthopedics, and healthcare management, giving her a comprehensive understanding of patient needs and operational excellence. Taylor is passionate about creating a supportive environment for both patients, families, and staff, ensuring that every aspect of care is delivered with compassion and efficiency.

 

Taylor’s commitment to Evergreen is driven by her strong belief in the need for healthcare accessibility for Maine’s seniors. She understands the unique challenges older adults face in rural communities and is dedicated to ensuring they receive the quality care they deserve. This mission fuels her leadership and inspires her work every day.

 

In her free time, Taylor enjoys reading, crafting, thrifting, and spending days at home with her Fiancé, beloved dog Dobby, cat Harper (a self-proclaimed nap consultant), and her lively flock of chickens.

Susan began her nursing career as a nursing assistant in Long Term Care, LONG before certification was needed or required, or even a concept! She completed her Bachelor of Science in Nursing in 1995 and worked as a hospice nurse in both rural and urban settings.

 

Susan returned to school in 2011 and obtained her MSN and became licensed and board certified as a Family Nurse Practitioner. She has worked inpatient and outpatient settings, including home visits, caring for all ages. She also holds board certification in hospice and palliative care.

 

Susan is an accomplished instructor, speaker, and author. She has presented at state, regional and national conferences and created curriculum for hospice and palliative care educational publications. She has served in multiple capacities in the Maine Nurse Practitioners’ Association and is currently on the board of Healthcare Advocates of Maine.

 

Susan enjoys life in Maine with her husband and 2 adult children who live close by. She enjoys living on the coast, sailing, hiking, kayaking, and time in the garden.

It was clear from a young age that she wanted to help people after seeing a caregiver ad in the local church bulletin although she was too young to help at the time. Dominique began her career working as a home health aide in an assisted living facility while attending nursing school. After graduating with a bachelors degree in Gerontology in 2005, she immediately enrolled in a nursing program so she could focus on elder care in her practice.

Dominique worked as a nurse in the ICU and PACU and medical oncology areas from 2008-2014. In 2014, she graduated from Upstate Medical University as a family nurse practitioner. As a nurse practitioner, Dominique has primarily focused on endocrinology and diabetes management along with acute hospital care.  She is active in the diabetes and endocrine community both nationally and locally. Her passion is bringing access to care to across all communities and areas in Maine.

 

Work and advocacy keep Dominique very busy. However, when home, she enjoys fixing up her 1890 farmhouse and spending time with her four dogs. 

She enjoys gardening and tending to her chickens on her homestead.

Leah Hayes, CNP

Leah came to nursing later in life after many years as a mother, wearing many hats. She earned her Master of Art in Clinical Mental Health with a concentration in Dance/Movement Therapy in 2003 which informs all her work professionally and personally. Leah fell in love with nursing after a career change in her 40’s. She worked as a certified nursing assistant for 3 years before earning her Bachelor of Science in Nursing followed by her MSN, becoming licensed and board certified as a family nurse practitioner in the state of Maine. She has spent her nursing career in long term care and hospice work, finding a deep passion for dementia and end of life care.

 

Leah continues to enjoy learning and attends multiple conferences and workshops every year pertaining to geriatric health care. She serves on the board of the Maine Nurse Practitioners’ Association and currently participates as a research assistant on a project to reduce rehospitalization rates at a skilled nursing facility in Portland.

 

Leah loves life in Maine with her son who is about to graduate high school and living near her adult daughter who performs frequently in theatre, music, and opera productions. She enjoys cooking, swimming, dancing, and a newly acquired passion for porch gardening.

Lynn Guilfoyle

Lynn grew up in Maine and now resides in Windsor with her husband. From an early age, she felt a strong calling to healthcare – a passion that has shaped her entire career.

 

Her journey began at 17, working in private homes and later at a boarding home on weekends while attending school. At 19, Lynn earned her CNA license and gained experience across diverse areas, including Mental Health, Med/Surg, and ICU. She also cared for her grandmother with Alzheimer’s, an experience that deepened her empathy and commitment to patient care. Determined to expand her skills, Lynn became a Certified Medical Assistant, a role she truly loves. Over the years, she has developed a profound compassion for every patient, finding meaning and value in every aspect of care.

 

Outside of work, Lynn enjoys spending time with her family, two married daughters and three grandchildren who bring her endless joy.
