Medical Director

Isaiah Meyer, MS, AGPCNP-C

Isaiah Meyer, MS, APRN, AGPCNP-C, is a board-certified Adult Gerontology Nurse Practitioner with over a decade of experience in geriatric medicine, complex care management, and healthcare leadership. He has worked in various healthcare settings, including academic programs and veteran’s health, specializing in geriatric primary care, chronic care management, dementia care, and home-based primary care.

 

Isaiah has served as a faculty clinician at Maine Dartmouth Geriatric Medicine’s Fellowship Program and VA Home Based Primary Care, where he excelled in interdisciplinary collaboration, clinical education, and program development. He is known for implementing innovative care models that enhance patient care and satisfaction.

 

Raised in New England, Isaiah earned his BSN from the University of Southern Maine and his MS in Nursing from Boston College. He has been actively involved in local and national professional organizations including geriatric board appointments, as well as health organization committee and association memberships.

 

Isaiah prioritizes patient experience, ensuring patients and caregivers feel heard. He believes in empowering older adults with the resources and education needed for quality care. Outside of work, Isaiah enjoys gardening, cooking, and spending time with his family and dogs in Central Maine.

Susan Folk, MSN, FNP-C, ACHPN

Susan began her nursing career as a nursing assistant in Long Term Care, LONG before certification was needed or required, or even a concept! She completed her Bachelor of Science in Nursing in 1995 and worked as a hospice nurse in both rural and urban settings.

 

Susan returned to school in 2011 and obtained her MSN and became licensed and board certified as a Family Nurse Practitioner. She has worked inpatient and outpatient settings, including home visits, caring for all ages. She also holds board certification in hospice and palliative care.

 

Susan is an accomplished instructor, speaker, and author. She has presented at state, regional and national conferences and created curriculum for hospice and palliative care educational publications. She has served in multiple capacities in the Maine Nurse Practitioners’ Association and is currently on the board of Healthcare Advocates of Maine.

 

Susan enjoys life in Maine with her husband and 2 adult children who live close by. She enjoys living on the coast, sailing, hiking, kayaking, and time in the garden.

Marjorie and her husband relocated to Portland from the Boston area 12 years ago. Upon arriving in Maine, Marjorie returned to school to pursue a career in nursing, with a specific focus on geriatrics—a population she has always been passionate about.

 

After starting her career in assisted living, skilled nursing, long term care, and memory care, Marjorie took a position with MaineGeneral Geriatrics where she worked on the inpatient units helping to prevent delirium in patients admitted to the hospital. While completing her Adult Gerontology Primary Care Nurse Practitioner program she trained under APPs and Geriatricians in the SNF, LTC, and palliative care settings. After graduation, she remained with MaineGeneral Geriatrics, seeing patients in assisted living, skilled nursing, and long-term care facilities as well as veterans in memory care. Most recently, she oversaw the care of 70 residents at Brentwood in Yarmouth, Maine, 60% of whom were in memory care.

 

Over the years, Marjorie has developed a deep interest and passion for supporting individuals living with dementia and their families. She is committed to delivering patient-centered care, prioritizing the unique needs and goals of each individual and their loved ones. She finds immense fulfillment in continuing to learn and grow in her work, and she is enthusiastic about bringing her skills and dedication to Evergreen.

 

Outside of work, Marjorie enjoys spending time at home with her husband and their two active boys. She loves reading, cooking, and exploring the woods with their Labrador retriever.
